Every potential patient will need to complete one of the following pre-educational items: watch our On-Demand Webinar, watch our Founder’s Video, take the 2-minute Right Fit Quiz, or read through our Blueprint to Health for Life email series.
We require this step to ensure your understanding of how our clinic cares for our patients. After one of these steps is completed, you can schedule a free 15-minute discovery call with our staff to determine if you are a good fit to meet with one of our providers.
Who is the Health For Life program best suited for?
Women and couples experiencing infertility, PCOS, hormone imbalance, or menopause symptoms—especially if they haven’t found answers or results with conventional care.
We also support individuals with hormone imbalances, gut/detox issues, inflammation, and low energy or metabolic concerns.
How is Health For Life different than other programs or office visits?
Our approach is a lifestyle approach. We combine 4 pillars – nutrition, detox, hormone balancing, and exercise.
Many other lifestyle programs do not address all of these issues and therefore, cannot be successful.
By addressing all 4 areas together, we have a system that corrects underlying root causes that are missed when one or more pillars are left out.
What age of patients do you serve?
Teens through individuals in their 70s. If a patient is a minor, it is required that they be accompanied by their parent(s)/legal guardian.
Do I need to live near Weston, Wisconsin?
No. We see patients both in person and virtually. We currently work with patients in Wisconsin, Minnesota, and Michigan.
What is the fee of the Health For Life program?
Fees will vary depending on the program length and individual medical needs. Fees for the particular plan of care will be determined in the initial consultation with the provider.
Do you accept FSA, or HSA?
We do accept HSA as a form of payment. We do accept FSA, and it will cover some of what Couri & Smyth Health For Life provides.
Do you accept referrals?
